Pre-departure Checklist
First, please print out and review the Pre-departure Checklist (Download PDF).
It contains critical passport, Husky ID, health, visa, and other form
due dates. All forms and a copy of your passport's ID page must be printed and faxed to Elizabeth Shaw at
617-373-8929 before June 1 unless otherwise noted. Contact Elizabeth Shaw at e.shaw@neu.edu for additional information.

- US Department of State Passport Renewal & Application Information - Important! If you currently have a passport that is due to expire before April 2009, you must renew it now.
